First Visit
to Endodontic and Periodontic Associates, Ltd.

Please assist us at the time of your initial visit to the office by providing the following information:

  • Your referral slip and x-rays from your referring dentist.
  • A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website.)
  • If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
  • Completed forms on the Patient Registration page.

Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).

We will complete an in-depth medical and dental health history and a thorough examination to measure for bone loss, loose teeth, bite, oral cancer screening, TMJ, and other signs of periodontal disease.

Will It Hurt?

We will be as gentle as possible.  Our doctors are trained in the most advanced techniques to minimize pain to little or no discomfort.

Do I Need X-Rays?

Yes, if your referring dentist has taken x-rays, you may request that they be forwarded to us, or  you may bring them with you.   In many cases, additional x-rays will need to be taken by Endo Perio.

What Will It Cost?

Fees for services, including evaluations, can vary based on the oral condition being addressed. When scheduling your appointment, our front desk associates will work with you to identify what costs, if any, may be the patient's responsibility.

Will My Insurance Cover The Cost?

At Endodontic & Periodontic Associates, many insurance plans are accepted. A list of all our in-network insurance companies can be found under the Insurance section of this website. Please be aware that individual plans vary. It is best to check with your insurance company to make sure of your benefits. Please bring all medical and dental benefit information and cards to your examination appointment.

Will I Need Surgery?

Not everyone needs surgery. If treated early, many cases can be controlled without surgery. We will make recommendations based on your individual situation. Our philosophy of practice is to treat as conservatively as possible to attain treatment goals.

Can My Teeth Be Saved?

The recent advances in endodontic and periodontal treatment allow us to successfully treat most teeth.

When Will I Go Back To My General Dentist?

Our office and your dentist will work closely together. If crowns and fillings are needed your dentist will provide them. Regular visits to your dentist are an important part of dental health.

What If I Delay Treatment?

Periodontal disease is a progressive, painless infection. Delay can cause you further bone loss and more expense. If your teeth are lost, dentures are never as effective as your own teeth.  If you delay endodontic treatment,

  1. The tooth would end up extensively decaying, leading to loss of tooth (extraction).
  2. The tooth could end up abscessing resulting in a hospitalization.
  3. The toothache could be so severe that time could be lost in the workplace.
