In-Network Insurance List

As of July 1, 2010, EndoPerio Doctors and Hygienists participate in the dental insurance plans listed below. Please note:

• We will gladly answer your questions about coverage and payment options.
• Dental insurance is not a guarantee of coverage. Dental insurance is unlike medical insurance and there are many limitations on what your insurance plan will cover.
• Endo Perio does not participate in HMO's, Medicare, or Medicaid.  If you have purchased supplemental Medicare coverage through any of the listed Endo Perio in-network insurance plans, please call us for further information.

EndoPerio is in-network with many different insurance companies. We will send claims to all insurance companies, in–network or out-of-network as a convenience to our patients. Insurance companies offer many different dental and medical plans. These companies can change benefits, co-pays, and deductibles many times throughout the year. Further, most dental insurance policies are limited and often only pay for a portion of the procedure(s) that may need to be done.

We do our best to provide you with accurate coverage estimates based on information available to us. At times, it is almost impossible to accurately estimate a patient's insurance co-payment without a specific treatment plan from the doctor and a phone call to your insurance company. Many insurance companies will not finalize the fees or their payments until after treatment is completed. Working with insurance companies can be difficult and time consuming. However, our staff members are happy to work with our patients to estimate financial responsibilities, process claims and establish patient payment plans when necessary. We offer multiple financing options including Care Credit for anyone that does not have insurance. As a courtesy, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current.

Co-Payment

The majority of dental plans reimburse approximately 30-80 percent of treatment costs. And, there are many limits in most dental plans. With this in mind, we generally ask that 30 percent of the fee be paid at the time of treatment.

Private & Group Insurance

As a courtesy to our patients with medical and/or dental benefit plans, we will submit necessary claim forms, receipts, and other information to your insurance company.

Upon receipt of an insurance payment, any balance due will be billed to you. If you have deposited an excessive co-payment, it will be refunded to you.
